Provide individual and group therapy to youth and adults using trauma-informed and culturally responsive approaches. Conduct assessments, treatment planning, documentation, and progress monitoring in compliance with ethical and regulatory standards. Support participants impacted by trauma, grief, community violence, justice involvement, and systemic inequities. Collaborate with case managers, credible messengers, and community partners to coordinate care and referrals. Participate in program planning, case conferences, and community-based outreach activities as needed. Maintain timely, accurate, and confidential clinical documentation.
The Case Manager is responsible for facilitating support and therapeutic groups and maintaining a caseload of clients who would benefit from individual psychoeducation and resource linkage. The case manager will assess client’s progress, needs to ensure appropriate community referrals occurs in a timely manner.
